How do I request an itemized bill?

You can request an itemized bill for most facilities by calling (888) 214-4066 and using the automated self-service system. You’ll need your account number, which can be found on your paper bill.

If you don’t have or don’t know your account number, you can speak to an agent to request the itemized bill. It will take about five business days to receive the bill in the mail.
