Following surgery and discharge from the hospital, most patients seamlessly transition to care in their homes provided by the Northwell Health Home Care Network. Once the patient has progressed with care at home, they are ready for outpatient rehabilitation services where the rehabilitation therapists assess and guide patients back to their highest level of function.
