Follow your heart

Patients’ health remains a priority, even after discharge. That’s why we created our exclusive Follow Your Heart program, a free and optional home care service aimed to eliminating hospital readmissions.

When a patient leaves our hospital, a member of the clinical staff who diligently cared for the patient during treatments will make follow-up home visits. The staff member, a specialty trained nurse or physician assistant, will be able to adjust medicine, perform routine health checks and additional services all in the comfort of a patient’s home.

Postoperative cardiac care

Our cardiac rehabilitation program provides comprehensive cardiac rehab programs to help people recover from heart attacks, bypass surgery, valve replacement/repair surgery, stent placement and additional heart and circulation problems. Using the latest technological advances in cardiac rehab treatment, the department's staff of renowned cardiologists and cardiac rehabilitation specialists provides safe and effective treatment plans that are individualized to each person's physical condition and treatment goals.

Major features of the cardiac rehababilitation programs include:

  • Medical supervision to offer the benefits of cardiac rehabilitation, improving the health and well-being of people who have heart problems
  • Cardiac rehabilitation exercise training, education on heart health and counseling to reduce stress and worry
  • Customized cardiac rehabilitation programs, designed for each person for a period of 12 to 36 weeks
  • Convenient morning and afternoon sessions

Cardiac rehab collaborates and coordinates with the following departments to provide the benefits of cardiac rehabilitation through a comprehensive multi-disciplinary approach to the prevention, diagnosis and treatment of heart disease:

Cardiac support groups

We connect patients with organizations managed by peers who have experienced cardiac surgery. The following organizations are able to provide additional support:

Mended Hearts, Inc./Mended Little Hearts is a national and community based organization that provides individuals who experienced heart surgery or heart disease an opportunity to encourage and support patients with similar conditions. The Mended Heart volunteers conduct hospital visits while a patient recovers, online check-ins and additional support through phone conversations. Learn more about this organization.
