Hospital-based care

After patients undergo surgical procedures, such as a joint replacement, but before they are discharged or enter inpatient rehabilitation, hospital-based physical therapy and occupational therapy services are available in Northwell Health hospitals. Hospital-based care is available at:

Inpatient rehabilitation

The rehabilitation professionals at the network’s inpatient facilities work in tandem with the rehabilitation network’s team of physiatrists and the patient’s primary physician to provide personalized services. By working with the patient’s PCP, they can offer a circle of care to make the most of the treatment process.

Acute inpatient rehabilitation

Designed for individuals who need frequent physician monitoring, the acute inpatient facilities provide 24-hour nursing and intensive physical, occupational and speech therapy as necessary to maximize recovery. The patient’s length of stay is based on diagnosis, needs and progress during treatment.

Acute inpatient rehabilitation services are available at the following locations:

Inpatient sub-acute rehabilitation

For patients who require less medically-supervised rehabilitation, the sub-acute inpatient facilities provide treatment until patients are ready to return home or to a long-term care setting. Depending on progress and direction of the patient’s physician, sub-acute treatment is often followed by home care or outpatient rehabilitation.

Inpatient sub-acute rehabilitation services are available at the following locations:

The Northwell Health Stern Family Center for Rehabilitation and Northwell Health Orzac Center for Rehabilitation have been awarded a five out of five rating for overall quality by the Centers for Medicare and Medicaid Services, placing both sites in the top 10 percent of nursing facilities in the country.

Outpatient rehabilitation

The teams of health professionals at the outpatient facilities across Long Island, Queens, Manhattan, Staten Island and Westchester will create an individualized treatment program based on each patient’s needs and goals – whether it is to speed recovery, decrease pain or simply return to the activities they love.

Outpatient rehabilitation services are available at the following locations:

  • Assistive technology professionals to help with wheelchair mobility and positioning.
  • Vestibular rehabilitation therapists to help reduce fall risk factors and resolve dizziness or vertigo due to inner ear or central neurological dysfunction.
  • Bioness-certified therapists help improve voluntary movement and re-educate muscles (e.g. foot drop and hand functions).
  • TPI (Titleist Performance Institute) Certified therapists. K-VEST equipment available
  • Pelvic floor rehabilitation to provide comprehensive management for patients with pelvic floor dysfunction, pain and to women with pregnancy and postpartum disorders.
