As part of a coordinated effort to ensure that patients get the support they need once they return home, discharge planners, case managers and social workers at Lenox Health Greenwich Village can make appropriate referrals to home care services, including nursing, therapy and home health aides.

Northwell Health At Home

For more than 30 years, Northwell Health At Home has been providing specialized home health care for patients needing medical assistance or therapy after an illness, injury, accident or surgery, as well as helping patients manage at home with chronic conditions, including asthma, congestive heart failure, stroke, diabetes and other illnesses.

The following medical professionals are available to assist patients and their loved ones:

Infusion services

Home infusion services from licensed pharmacy and nursing professionals are available for patients in need of treatment for pain management and infections, as well as those requiring hydration, nutrition and chemotherapy services.

Geriatric care management services

The Circle of Care program is a fully integrated care solution that assigns each client a specific geriatric care manager to coordinate all aspects of an individualized care plan. Geriatric care managers are licensed social workers who specialize in meeting the needs of seniors who require personalized services. The care manager helps arrange and coordinate community-based and health related services to assist clients so they can continue to live safely and comfortably in their own homes. Geriatric care managers provide a range of services to clients and families, including health advocacy, home health needs, transportation, elder law and financial planning.

Assistance is available for residents with personal care and basic homemaking tasks, ranging from a few hours a day to around-the-clock care.
