About us

The central assessment and intake services unit at Staten Island University Hospital offers comprehensive evaluations to determine if an individual is in need of chemical dependency treatment. The final section of this biopsychosocial evaluation recommends clinical intervention when appropriate.

Finding available services

All level of care decisions are based on the patient placement criteria defined by the American Society of Addiction Medicine (ASAM). If the appropriate services are not available at Staten Island University Hospital, a referral will be made to another provider. Assessments are carried out by physician assistants and chemical dependency counselors with many years of experience working with addicted individuals.

Assessments and interventions

There may be occasions when an individual or family member feels uncertain whether or not a problem exists with alcohol or other drugs. In such a case, we offer an assessment for a nominal fee. A report summarizing the findings and any recommendations will be mailed to the responsible party within seven days contingent upon a proper Release of Information being signed by the individual. Federal confidentiality regulations 42 CFR Part 2 applies to all individuals (including minors). Please note that this service is generally not covered by insurance.

The central assessment and intake services unit is located at 392 Seguine Avenue, Staten Island, NY.

Please call (718) 226-2800 in advance to schedule an appointment, but walk-ins are also welcome.
