The Sliding Fee Scale Program is available to uninsured residents of the various service areas of the Northwell Health hospital facilities who are ineligible for public health insurance. This program is for assistance paying for multiple treatments, services or ongoing care. Applicants must also meet income requirements.

Income guidelines

The income restrictions for program eligibility are set based on the federal poverty level and a patient’s household size. To determine your eligibility, consult the table below.

Program restrictions

The following items are NOT covered under the Sliding Fee Scale Program:

  • Prescriptions
  • Co-pays
  • Co-insurance
  • Deductibles
  • Personal items (e.g., telephone, private room differential, guest meals, etc.)

Frequently asked questions

I am uninsured and need to find out about where I can go to receive low-cost health care services. How can Northwell help me?

Northwell has a Sliding Fee Scale Program that provides routine medical care at a low cost. Eligibility for the Sliding Fee Scale Program is based on your household income and residency. Please review our income guidelines to see if you may be eligible. Patients, if eligible, must first apply for Medicaid or other public insurance. We can help you with your application.

Can you tell me more about the household income requirements?

To be eligible for the Northwell Health Sliding Fee Scale Program, you must live in a household with an annual income under 500 percent of the Federal Poverty Level. Please see our income guidelines to see if you qualify. Call or visit our ambulatory care centers to make an appointment to see a doctor and apply for our program.

OK, I meet the income requirements. What else does Northwell look at when reviewing my request to participate in the Sliding Fee Scale Program?

We look to see if you live within the Northwell service areas, and sometimes we will have to ask you questions about your financial assets.

What charges are NOT covered by the Sliding Fee Scale Program?

Only services that your doctor thinks are medically necessary will be considered for the program. Prescriptions and personal items (e.g. telephone, private room differential, guest meals, etc.) are NOT covered under the Sliding Fee Scale Program.

If I need to have ambulatory medical procedures/tests, how much will I be responsible for paying?

As an established Sliding Fee Scale Program patient, you will always be entitled to reduced fees for medically necessary services. For ambulatory services/tests, the registration representative or financial counselor will advise you on your responsible amount before your scheduled procedure/test date. You will then be able to make a financial arrangement before your scheduled visit.
