National - Medicare Innovation
Bundled Payments for Care Improvement (BPCI)
Medicare’s full-risk, episode of care demonstration program in several Northwell Health hospitals coordinates post-discharge care for patients who were hospitalized with Chronic Obstructive Pulmonary Disease, Hip/Knee Joint Replacement, Cardiac Valve Replacement, and Coronary Artery Bypass Surgery.
Pioneer Accountable Care Organization (ACO)
In partnership with Montefiore Medical Center's Pioneer ACO and the University Physicians Group (UPG) in Staten Island and Brooklyn, Northwell Health Solutions care managers follow up with patients after hospital discharge and help coordinate their care.
Independence at Home (IAH)
Northwell Health Solutions House Calls Program provides home-based care for frail, chronically ill patients who have difficulty getting to doctor's office. The focus is on forming long term relationships with patients and their caregivers, understanding their personal wishes about their care, and preventing hospitalization when frail patients prefer to be cared for in the comfort of their own home.

New York Health Home
Northwell Health Solutions is the state designated lead health home provider for Nassau, Suffolk, and Queens counties serving Medicaid recipients with complex needs. Click here to learn about health home for children.
New York Delivery System Reform Incentive Payment (DSRIP) Program
DSRIP is a program to increase access to high quality care for Medicaid beneficiaries and the uninsured, with the goal of reducing potentially avoidable admissions by 25 percent. Providers, including hospitals, physician groups, and skilled nursing facilities, are incentivized to collaborate on agreed-upon projects, including evidence-based disease-management and readmission-reduction programs.
Commercial Health Insurance Payers
Northwell Health has arrangements with national and regional insurers in pay for performance and risk programs. Northwell Health Solutions assists community physicians by streamlining quality measures and providing an array of care management services.
The Healthy Transitions Program in Late Stage Kidney Disease
This patient-centered program aims to improve CKD patient outcomes by coordinating care, preventing avoidable hospitalizations, and identifying opportunities for dialysis or transplantation.
Northwell Health Solutions launched this program as a partnership between Northwell HealthCenter for Emergency Medical Services (CEMS) and the House Calls Program for frail patients that have difficulty leaving their home. Paramedics respond in-person to those patients' urgent needs in consultation with a primary care physician via telephone or, if available, videoconferencing.
