NS-LIJ Creates ACO for 1199 Members
North Shore-LIJ and 1199 SEIU announced yesterday that the hospital system will manage care for about 10,000 union members through a new accountable care organization that will focus on those who have chronic conditions or are recovering from hospital stays. The program takes effect Jan. 1 and will coordinate care for members through the North Shore-LIJ Premium Network, which includes about 5,500 clinicians in the New York area. Diabetes, hypertension and asthma are three of the most common chronic conditions facing 1199 SEIU workers, according to Mitra Behroozi, executive director of the union's benefit and pension fund. The union began an accountable care agreement with Montefiore Health System in July 2014, and is currently in talks with Mount Sinai Hospital about creating a similar program, said Behroozi. "We are actively trying to pursue as many opportunities as we can" to implement value-based care, she said. She added that it's too soon to evaluate whether the Montefiore agreement generated savings.

COORDINATED CARE — The 1199SEIU Benefit Funds, 1199SEIU United Healthcare Workers East and North Shore-LIJ Health System are creating a coordinated care program for about 10,000 members. The program begins Jan. 1. Care for 1199SEIU members will be coordinated through North Shore-LIJ Premium, an integrated network of more than 5,500 primary care and specialty physicians as well as other healthcare professionals in the New York metropolitan area, according to a press release from North Shore-LIJ. “This new collaboration with 1199SEIU marks the first time the health system and a union have entered into a value-based contract that allows us to focus our efforts on improving important patient outcomes by enhancing the coordination, delivery and efficiency of care to our health care colleagues,” Michael J.
