GREAT NECK, NY – If you need to make an appointment or get copies of your lab reports, you don’t have to wait until 9 a.m. to call a doctor’s office. To help patients take a more active role in their health and wellness, the North Shore-LIJ Health System has rolled out a new, web-based patient portal that allows patients to access their personal health information from the comfort of their home or elsewhere, at any time.
FollowMyHealth ™ is available to patients who have received care from North Shore-LIJ hospitals and physician practices across Long Island and in New York City at http://fmh.northshorelij.com/. FollowMyHealth™ is a universal health record that can be accessed on any computer, tablet or smart phone 24 hours a day, 7 days a week. FollowMyHealth™ gives you the ability to:
• Review your personal health information online
• Communicate privately with physicians for non-emergencies, via secure messaging
• View test and lab results as well as your doctor’s comments about these results
• Review your list of medications and request prescription refills
• Send requests to schedule or change appointments
• Update your health information, such as allergies, medications, etc.
• Review discharge instructions, including medications, after a hospitalization
There are two ways to connect to the free portal. The first is by the patient responding to an email invitation, which is automatically sent after an office visit or hospital discharge. The patient can then create an account with a user ID and password and immediately begin using the portal.
The other way to get a FollowMyHealth ™ account is to self-register at:
https://northshorelij.followmyhealth.com. Patients can create an account and request a connection to their North Shore-LIJ medical records. However, due to the need to carefully review and verify these requests, it may take several days before the health information is available online for viewing. To use FollowMyHealth ™, a patient must be over age 18. However, the portal offers a proxy feature for parents who wish to manage their child’s health information or caregivers for patients unable to manage their own account.
According to Rosalie Long, RN, vice president of practice innovations at the North Shore-LIJ Medical Group, approximately 4,000 patients across the health system have already registered for FollowMyHealth ™ and almost 100 people a day are connecting to the patient portal.
“Our goal is to connect as many patients as possible to FollowMyHealth™, with the potential of reaching hundreds of thousands of people,” Ms. Long said.
“This is a seamless and informative way for people at home to have their health information available at their fingertips,” said John Bosco, chief information officer for North Shore-LIJ. “As we continue to roll out additional functionality in the portal over the coming year, we will make it even easier and more convenient for our patients to stay connected and obtain the information and services they want from us.”
