GREAT NECK, NY –– The 1199SEIU Benefit Funds, 1199SEIU United Healthcare Workers East and North Shore-LIJ Health System today announced the launch of the 1199SEIU North Shore-LIJ Coordinated Care Program. The new program will coordinate care for some 10,000 1199SEIU members.
“Our members spend so much time taking care of patients that they don’t always find time to take care of themselves,” said 1199SEIU President George Gresham. “Our new partnership with North Shore-LIJ gives 1199SEIU members the extra support they need to stay healthy with more streamlined care. This is a major step in the right direction for the industry, because quality healthcare depends on healthy caregivers.”
The program takes effect January 1, which coincides with the health system’s name change to Northwell Health. Care for 1199SEIU members will be coordinated through North Shore-LIJ Premium, an integrated network of more than 5,500 primary care and specialty physicians as well as other healthcare professionals in the New York metropolitan area. They will deliver accessible, efficient, team-based, integrated care focused on the individual needs of eligible 1199SEIU members. Members can continue to access North Shore-LIJ’s network of 21 hospitals, approximately 450 outpatient practices and a full continuum of rehabilitation, long-term care, home care, hospice and other services on Long Island and in New York City and Westchester County.
“This new collaboration with 1199SEIU marks the first time the health system and a union have entered into a value-based contract that allows us to focus our efforts on improving important patient outcomes by enhancing the coordination, delivery and efficiency of care to our health care colleagues,” said Michael J. Dowling, president and chief executive officer of North Shore-LIJ. “This accountable care agreement is yet another example of our health system’s shift away from episodic, fee-for-service medicine to a performance and value-based delivery model that allows us to align quality care with payment incentives.”
The new program will create care management teams that can develop a personalized plan to help members manage a chronic condition, recover from a serious illness or successfully transition from a hospital stay to an aftercare setting. Through this patient-centered approach, the program also will help reduce unnecessary hospital readmissions and support individual efforts to improve overall health, while also reducing costs.
